Limited Flu Vaccine For Hackensack Residents To Be Distributed By Lottery
The Hackensack Health Department will be holding a lottery to distribute the limited amount of influenza vaccine, 190 doses, that the department anticipates it will receive this year. Lottery applicants must be Hackensack residents and must fall into the CDC’s priority groups for receiving influenza vaccine this year, which include, those who are 65 years of age and older or those who are 2 to 64 years of age with an underlying medical condition that warrants receipt of the vaccine. Proof of residency, age and underlying medical condition, if applicable, will be required. Application forms may be picked up at the following locations, Hackensack Housing Authority Senior Citizen buildings located at 65 First Street, 164 Beech Street and 60 Kansas Street. Applications will also be available at City Hall, the Health Department, Johnson Public Library and here on this web site:

Applications must be submitted in person at the Hackensack Department of Health, 215 State Street, Hackensack. The application deadline is Tuesday, November 30, 2004.
